The measure, the form, the report
Each assessment-type folder carries assessment.json: the exact Likert items, the reverse-scoring, the interval. This guide follows it end to end — into a running form on FormLM, through both passes in the room, out the other side as a delta report you can hand a manager.
Step 1 — the spec is already written
Open any assessment-type bundle; *-assessment.json is the machine-readable version of the Kirkpatrick Level-2 question — did anyone actually learn anything — phrased as items. Six items for pairs exercises, four for name games, eight-plus-follow-up for training evaluation. Nobody guesses at the wording; the folder is the wording.
Step 2 — generate the live form
formlm-cli, the consent gates
npm install -g @formlm/cli formlm-cli auth login # free account; headless? see the skill's # fallback: hand this one line to a human formlm-cli smart generate --input "<folder's own prompt, in SKILL.md>" # the form and every response stay in YOUR workspace — # not the workshop site's, not the agent's
Step 3 — two passes, one delta
Pre-measure before pairing up (2 minutes), post-measure after the debrief. Same items, same order, anonymous links. The value FormLM adds over the paper sheet is not the collection — it is that each participant's before/after difference is computed automatically, and the report shows the pair- and room-level picture without ranking anyone.
Step 4 — the report, honestly
Deltas are self-report: good for detecting movement, bad for absolute claims — the guides say so plainly (pre/post-measures, Kirkpatrick). Use the report the way the folder intends: to compare sessions, to defend the 15 minutes you spent on the debrief, to answer the sponsor's email with a chart instead of a smile sheet.

questions
Do participants need accounts?
No — they open a link and answer. The account is yours (free), holding your forms and your data.
Can I run it fully on paper instead?
Yes — every folder includes the printable answer sheet; the delta math is the guide's two-pass arithmetic, no login anywhere.
Is the report enough for TTQS/QF paperwork?
It is Level-2 evidence with timestamps; the TTQS guide on this site maps which artifacts the paperwork expects, and which of them these folders produce.
