Workshop Feedback Form
A 90-second exit ticket for public workshops
Ratings are a health check. The real products are permissioned testimonials and a voted topic list for your next event — ninety seconds at the door.

of your workshop actually come from?
WorkshopSkill · Workshop Feedback Form — Slide Deck · CC BY 4.0Why an exit ticket
- No HR department chases non-responders at a public workshop. Past the door, feedback is gone.
- So the form is part of LEAVING, not homework: box at the door, poker card in hand, ninety seconds.
- “A quick form by email tomorrow” rounds to zero for public events. Never promise it.
What is on the card
- Four health-check rows: promise kept, pace, working-not-listening, standalone usefulness.
- One 0–10: would you recommend this to a colleague?
- Two open lines: the one-sentence testimonial, and the topic you want us to run next.
The quote permission box
- “You may quote this — first name only.” One checkbox keeps the form honest.
- Anonymous by default; quotable by choice. Chosen quotes are sharper sentences.
- Never edit a quote beyond typos. Too vague to use? Ask the person — do not sharpen it yourself.
The line that converts
- Say it while pointing at the box: “Drop it on your way out — it decides what I run next.”
- That last clause is true. It is the whole ask.
- Item 4 is the trust row: useful-without-upsell. The moment the room smells a funnel, scores and quotes decay together.
The 60-second read, ten minutes per event
1. Median recommendation: 8.5+ sells itself; below 6 — read the rows before the quotes.
2. Quotable sentences: named action, before/after, a number. Two is a good harvest.
3. Top demanded topic: one repeat is a candidate, three repeats is your next event.

- Publish the top three back to that room: “you asked — here is next quarter.”
- A year of exit tickets is a catalog that writes itself: quotes become the page, demand becomes the calendar.
