Post-Training Survey
A 5-minute feedback form that goes one step past the smile sheet
Satisfaction is easy to collect. The question sponsors actually ask is: will anything show up in the work? This survey is built to answer it.

What did it say about Monday?
WorkshopSkill · Post-Training Survey — Slide Deck · CC BY 4.0Where smile sheets stop
- Kirkpatrick’s Level 1 measures reaction — pleasant, easy, nearly information-free.
- Most evaluation stops there because satisfaction is safe to collect and nice to report.
- A smile sheet can only go up. This survey can tell you what to fix.
What the eight questions measure
- Six rows: relevance, transfer intent, pace, facilitation, clarity — and one blocker.
- Item 2 is the bridge: “I can name one thing I will use within two weeks.”
- Two open questions: the Monday commitment, and the honest obstacle.
The reverse item is the lie detector
- Item 6: “Nothing from today will change how I work.” Low is good.
- Polite rooms score everything 4 — including “nothing will change.”
- When item 6 rides high while item 2 rides high too: trust item 6.
How to run it
- The last five minutes, in the room: 90%+ response. “Later by email” donates your best window to nobody.
- The anonymity promise, said out loud in your own words, before the cards go out.
- Read item 2 and the open answers BEFORE any averaging. Averages are the last stop, not the first.
The 60-second read
1. Item 2 at 3.5+? Below 3.0, the session was theater — and the design is the suspect.
2. Countable Monday commitments — concrete verbs, or vibes?
3. An obstacle named twice is a finding. Named three times: a redesign order.
WorkshopSkill · Post-Training Survey — Slide Deck · CC BY 4.0The follow-up is the survey’s second half
- Every concrete Q7 answer becomes one 30-day line: “You said you would — how did it go?”
- No tooling, one message, honest replies: the cheapest Level 3 check in existence.
Numbers tell you where to look.
The open answers tell you what happened.
Report the lowest item, always.
